The birth record should be in English and include information on the name of child, date of birth, place of birth, name of mother, name of father and sex of child.<\/li><li>Original and uncertified photocopies of both parents\u2019 national identification card.<\/li><li>Original and uncertified photocopies of parents\u2019 marriage certificate<\/li><li>Please note that the service is free for children under the age of ten (10).
