REQUIREMENTS
- Completed BD3 form – it will be given to you at the Embassy upon arrival
- Original and uncertified copy of child’s birth record (ie extract from Skatteverket in Sweden). The birth record should be in English and include information on the name of child, date of birth, place of birth, name of mother, name of father and sex of child.
- Original and uncertified photocopies of both parents’ national identification card.
- Original and uncertified photocopies of parents’ marriage certificate
- Please note that the service is free for children under the age of ten (10). The cost at the Embassy for children over the age of ten (10) is 200 Swedish kronor in cash and in exact amount.
- Please note that it is recommended that both parents are present at the Embassy to sign the BD3 form to avoid queries when submitting the form in Zimbabwe.
- A single mother can also apply for the child’s birth certificate by producing originals and uncertified photocopies of her documents as stated above.
