Passport Application

Passport forms are available at the Embassy. Please be advised that both the Embassy and the Registrar General’s Office do not accept downloaded passport form.

REQUIREMENTS FOR ADULT APPLICANTS

1. Original long birth certificate and uncertified photocopy
2. Original Zimbabwean national identity card, synthetic polythene, metal or green waiting pass with a photograph and uncertified photocopy for applicants who are aged 16 years and over. Driver’s Licence are NOT ACCEPTED.
3. Original and uncertified copy of marriage certificate/divorce order (if applicable)
4. Two identical colour photographs (size 3.5cm x 4.5cm) on a strictly white background with applicant wearing dark clothes, no sunglasses or spectacles and no jewellery.
5. Original expired or current passport and uncertified photocopy of photo and details page
6. Original Police Report is required for all lost or stolen passports. Must be translated in English
7. Letter of authorisation which authorises a relative/nominee in Zimbabwe to submit and collect the passport for you. The letter should have full name and ID number of both the applicant and the relative/nominee.
8. Cost at the Embassy: 200.00 Swedish Kronor in Cash and in exact amount per application.
9. A personal appearance by all applicants at the Embassy is required for fingerprinting and authentication of documents.

REQUIREMENTS FOR APPLICANTS UNDER 18 YEARS

1. As above. The following is also required:
2. Original national identity document and uncertified photocopy for children who have attained the age of 16 years
3. The father or mother must be present to sign ‘Section 5’ of the application form. They should bring original and uncertified photocopy of Zimbabwean birth certificate, national ID and current passport as well as resident permit.
4. In the event that both parents are deceased, original death certificates and uncertified copies are required.
5. Legal guardian’s certificate or documentary proof should be in the form of a Zimbabwean Court Order.
6. Documents such as certificates of guardianship must be authenticated by the Ministry of Foreign Affairs of the country which issued it.

ADDITIONAL PASSPORT APPLICATION INFORMATION

1. A personal appearance at the Embassy is REQUIRED for fingerprinting and authentication of documents. It cannot be done at a police station or by a Notary Public.
2. The application forms cannot be mailed to you. They will be issue to you upon arrival at the Embassy.
3. When making arrangements to come, please be advise that the average time it takes to complete the forms and process the documents is 45 minutes per person. It may take longer if you do not have all the required documents and/or correct passport photos.
4. We strongly encourage you to bring photocopies of the required documentation that will be authenticated at the Embassy. This will greatly reduce processing time and duration of visit to the Chancery.
5. Once your forms have been processed and documents authenticated at the Embassy, everything will be returned to you. You must send your forms to the next of kin/the person you have given authority to submit and collect your passport application. Cancellation of passport is either done at the Embassy or in Zimbabwe depending on what one prefers. Cancellation of passport at the Embassy is for free.
6. Please be informed that the processing and issuance of passports are done in Zimbabwe. The applicant is responsible for making their own arrangement for shipping of their old and new passports to and from Zimbabwe.
7. Passport application forms expire 6 months from the date of completion. Ensure that your complete application package is received by the Passport Office in Harare before the forms expire.
